Office Policies

Confirmation Fee

  • New patients and/or returning patients not seen in the past 6 months will be required to pay a confirmation fee of $25.00 when appointment is made. This fee will be applied to your first visit.

Copayments, Balances, Information Changes

  • Copayments and balances are due prior to seeing provider.
  • Please notify us of any changes of address, phone number, or Insurance.

Prescription Fees

  • $20.00 for Email
  • $25.00 for Pick-up
  • $30.00 for Call-in
  • $25.00 for Prior Authorization. Must be paid in advance.
  • We do not call Prescriptions in after hours.

Late Cancellation / No Show Fees

  • 24 hours notice must be given for cancellations, or you will be charged.
    • Dr. Sherbondy and Donna Ambrose — $60.00
    • Therapist — $80.00

Disability Forms

  • This office does not fill out Permanent Disability Forms.
  • If Dr. Sherbondy takes you out of work, Short Term Disability Forms can be completed for a fee, which must be paid prior to the form's completion.

On the day of your visit, please be sure to bring the following items.

  1. Insurance Card(s) and
    (if your insurance requires) Prior Authorization letter or number

  2. Picture ID (Driver's License)
  3. All completed forms available on this web site if you choose to complete them prior to your visit

Please arrive 15 minutes prior to your appointment to allow proper time to check in at the front desk.
