New Patient Self Referrals: Please complete the Patient Self-Referral Form in its entirety. You can then mail, fax or email the form to us along with a copy of your insurance card FRONT and BACK. Once we receive the referral, our referral specialist will give you a call to schedule your appointment. We will need a copy of the patient's insurance card FRONT and BACK along with any pertinent medical records. You may fax or email these to us. Once we have received the referral, our Referral Specialist will give the patient a call to schedule his or her appointment. Once that appointment has been made, you will get a fax or an email letting you know this appointment is confirmed for the patient in which you referred.
